Avian Influenza: First information from the joint WHO/OIE/FAO mission

Dr. Veronique Jestin from the OIE Reference Laboratory in Ploufragan, France,
and OIE expert, has returned from her mission to Viet Nam with the
WHO/OIE/FAO field team. In collaboration with the Vietnamese veterinary
authorities and the other members of the mission, she conducted an evaluation
of the current epidemiological situation relating to avian influenza in that
country.
On 23 Jan 2004, the situation described by the Vietnamese authorities was
23 affected provinces, 445 outbreaks, and 2.9 million affected poultry.
The conditions in the north of the country are described as under control,
while in the south the epidemic appears to be developing further.
Among some mixed establishments containing pigs and poultry that are
infected, only the poultry are being culled.
The Vietnamese authorities have officially requested that the OIE continue
to provide advice on defining the policies required to manage such a
situation.
The introduction of sentinels before any repopulation, as well as the
possible use of a systematic prophylactic vaccination, were discussed.
Furthermore, the Vietnamese authorities have agreed to continue to work in
very close collaboration with the experts from the WHO, the OIE, and the FAO.
The WHO/OIE/FAO operational teams continue to operate in the affected
countries of the region and to provide their recommendations in full
transparency to the concerned governments.

[Information from Vietnamese authorities on 21 Jan 2004 indicated 2.5
million birds in 19 provinces were affected. The information above
indicates further spread but not at an exponential rate. An important
surveillance node is the pigs on the mixed farms highlighted in this
report, as pigs can reassort the viral genotype and foster the emergence of
new human strains.
The kind of information contained in this report is extremely valuable. The
transparency of the Vietnamese health authorities should be emulated by all
countries in the region, as it represents our best chance of avoiding
pandemics in animals and humans. - Mod.PC]

Avian influenza in Laos

Information received on 27 Jan 2004 from Dr Singkham Phonvisay, Director
General, Department of Livestock and Fisheries, Ministry of Agriculture and
Forestry, Vientiane:
Date of the report: 27 Jan 2004.
Nature of diagnosis: clinical, post-mortem, and laboratory.
Date of initial detection of animal health incident: 19 Jan 2004.
Estimated date of primary infection: 14 Jan 2004.
One outbreak was reported in a 3000-head poultry layer farm in the village
of Nonsavang, close to Vientiane.
Total number of deaths: 2700.
Total number of birds destroyed: 300.
Diagnosis:
A. Laboratories where diagnosis was made:
- National Centre for Veterinary Diagnosis, Hanoi, Vietnam (on 26 Jan 2004);
- Northeast Veterinary Research and Diagnostic Centre, Kon Khaen, Thailand
(on 27 Jan 2004).
B. Diagnostic tests used: inhibition haemagglutination test.
C. Causal agent: avian influenza virus subtype H5. Further tests are
underway to confirm whether the virus is identical to the H5N1 which is
causing the outbreaks in neighbouring countries.
Source of agent / origin of infection: unknown; under investigation.
Control measures:
- stamping out,
- quarantine,
- movement control,
- active surveillance and culling programme,
- regional coordination through the FAO (Food and Agriculture Organization
of the United Nations).
