Viet Nam: avian influenza situation - WHO update 4

The Ministry of Health in Viet Nam has confirmed a new case of human
infection of H5N1 avian influenza. The case has been confirmed by the
National Institute of Hygiene and Epidemiology (NIHE).
The case is a 23-year old female from Cam Khe district, Phu Tho
province. She developed symptoms on [14 Feb 2008] was hospitalized on
[19 Feb 2008] and died on [25 Feb 2008]. The case had contact with
sick and dead poultry prior to her illness.
Of the 105 cases confirmed to date in Viet Nam, 51 have been fatal.

[This is the confirmation report from WHO on the latest death
attributable to avian influenza (H5N1) infection in Viet Nam. It is
the 51st fatality in Viet Nam since 2003. The table of cumulative

Viet Nam's National Institute of Infectious and Tropical Diseases is
treating 2 people from northern Hung Yen and Thai Nguyen provinces,
who have developed bird flu symptoms, according to local newspaper
Pioneer on Wednesday [27 Feb 2008].
Specimens from a 23-year-old person from Hung Yen and a 48-year-old
person from Thai Nguyen, who currently need respiratory assistance
because their lungs have been severely damaged, are being tested for
bird flu virus strain H5N1, the newspaper quoted Nguyen Hong Ha, vice
director of the institute in Hanoi, as saying.
Initial investigation has shown that the 2 patients had contacted
with fowls before exhibiting bird flu symptoms.
To date, Viet Nam has confirmed a total of 105 human cases of bird
flu infections, including 51 fatalities, in 35 cities and provinces
since bird flu started to hit the country in December 2003.
Viet Nam currently has 8 localities having poultry being hit by bird
flu: Thai Nguyen, Quang Ninh, Hai Duong, Nam Dinh, Tuyen Quang, and
Ninh Binh in the northern region, and Long An and Vinh Long in the
southern region, the country's Department of Animal Health said [26 Feb 2008].

[The newswire in section [2] above describes 2 new suspected cases of
avian influenza infection in residents of northern Hung Yen and Thai
Nguyen provinces. Both suspected cases have a history of contact with
poultry before the onset of symptoms that were described as
compatible with H5N1 infection, although laboratory tests were not
available as yet.
The rapid confirmation of the suspected case in Phu Tho (see WHO
confirmation in section [1] above), has one wondering if the other 2
pending suspected cases -- a 7-year-old boy from Hai Duong province
that was reported by a newswire to have been confirmed by the
national laboratory to have H5N1 infection, and a newswire report of
a suspected case in a 40-year-old man from Bac Lieu province who died
from a pneumonia illness with a history of contact with sick poultry,
but where no information on laboratory testing was provided -- were
ruled out as being due to H5N1 infection, since they have been
"pending" for some time now. Clarification of the status of these
suspected cases reported in the lay press would be greatly
appreciated by PRO/MBDS.
The newswire above [2] with information on yet another 2 suspected
cases of human infection with avian influenza virus that are being
tested, combined with the earlier reports of suspected but as yet
unconfirmed reports of human cases leads one to conclude that the
local media are highly sensitized to the possibility of human
infections with the H5N1 virus and are therefore "reporting"
suspected cases before laboratory confirmation is available. Given
that heightened awareness on the part of the local media, if these
pending 4 suspected cases are not confirmed to have H5N1 infection,
it may be a signal for us at PRO/MBDS to await laboratory
confirmation of H5N1 infection before posting newswires with
"suspected" cases so that we do not fall victim to "sensationalist" reporting.
