Up to 21 May 2009, 10 540 confirmed cases of the new virus influenza
A (H1N1) infection, including 86 deaths, have been notified in 14
countries of the Americas: Argentina, Brazil, Canada, Chile,
Colombia, Costa Rica, Cuba, Ecuador, El Salvador, Guatemala, Mexico,
Panama, Peru and the United States. The date of the onset of symptoms
of the 1st confirmed case was [28 Mar 2009] in the United States.
WHO is not recommending any travel restrictions related to the
outbreak of the Influenza A (H1N1) virus.
The new virus influenza A (H1N1) is sensitive in vitro to
Oseltamivir. However, it should be emphasized that the knowledge of
the safety and efficacy of this drug comes from the experience in the
treatment of seasonal influenza.
In the event of confirmed or suspected cases of infection by the new
virus influenza A (H1N1) that fulfill clinical criteria of severity,
the treatment with Oseltamivir has to be initiated immediately.

The report contains graphs and charts detailing the epidemic curve,
variables related to age specific morbidity and mortality, geographic
distribution, clinical presentation of fatal cases and population
based attack rates. A close look at the epidemic curve presented in
this update reveals one laboratory confirmed case on 11 Mar 2009 (as
the earliest confirmed case), and the most recent date of onset of
symptoms with laboratory confirmation with 2 cases on 18 May 2009.
The peak number of confirmed cases on a daily basis occurred on 26
and 27 Apr 2009. The overall case fatality rate (CFR) is now down to
1.9 percent (in the 1st report on CFRs on 6 May 2009 the overall CFR
was 3.8 percent). Pre-existing illnesses among the fatal cases
included: metabolic abnormalities such as diabetes and/or obesity
(29.3 percent), cardiac disease (13.3 percent), smoking history (10.7
percent), respiratory disease (5.3 percent), infectious diseases (4.0
percent), autoimmune diseases (1.3 percent) and malignancies (.3

[To summarize the current situation, as of 6:00 AM GMT on 21 May 2009
there have been a total of 11 034 cases and 85 deaths of influenza A
(H1N1) infection officially reported to WHO coming from 41 countries,
up from 10 243 confirmed cases and 80 deaths from 41 countries
yesterday (20 May 2009). According to the WHO update, there were no
new countries reporting cases in the past 24 hours. According to
newswires, the Philippines have confirmed a 1st case of influenza
A(H1N1) infection in a 10-year-old girl with a history of travel to
the USA and Canada. Of note is the newswire with confirmation of a
1st case in Honduras with no history of travel. One suspects that
there is more widespread distribution of the influenza A(H1N1) virus
around the world than has been officially confirmed, especially when
many locations have implemented active surveillance of cases among
people with known epidemiologic links to locations where the virus
has been documented.
The USA has officially reported 5764 laboratory confirmed cases
coming from 48 states (compared with 5710 cases from 48 states on 20
May 2009), and 9 deaths. Mexico has reported 4008 cases and 78 deaths
(compared with 3892 cases and 77 deaths on 20 May 2009). Canada has
reported 719 cases and one death (compared with 496 cases and one
death on 15 May 2009).
